Application Process
The Day Treatment application is a two-step process.
First, please complete the application form by following the link below.
Upon completing the application return to this page and locate the link to fill out the Release of Information (ROI) forms needed.
Before we can process an application, we do need to obtain a current diagnostic assessment (within the last six months) and current academic records. Signed releases are needed to obtain any records from schools, care providers, and other medical professionals.

Please return to this site upon completion of the application and follow the link for Release of Information for your application to be reviewed.

Step 2: Release of Information
Before we can process an application, we need to obtain a current diagnostic assessment (within the last six months) and current academic records.
Please note, if your child has not had a diagnostic assessment within the last six months, Youable may reach out to you to schedule an intake appointment to determine their eligibility for our program.
Signed releases are needed to obtain any records from schools, care providers, and other medical professionals. Please fill out the Release of Information form so we may obtain a copy of your child's academic records, which may include an IEP, behavioral plans, attendance records, and more.
An additional form will be needed if your child recently had or is currently seeking treatment for their mental health care, specifically for the provider and location. Once the release is received, we'll reach out to obtain the necessary records.
